Glock 17, running my handloads.   Was when i was using a single stage press.   Some how some way I had a round that received extra powder.   Went shooting.   A fireball came back out of the pistol towards my face, blew the nearly full magazine out of the pistol on to the floor and stung my hand something fierce.   The 9mm case had ruptured right at the extractor groove for about 3/4 around the cartridge.   Scared me.  Luckily the pistol took no damage - amazing.   Checked everything out and went back to shooting.   The one and only time I had this happen.

then I was running 115 gr bullets with Win 231.   Now I run 124/125 gr bullets with Win 231.   Not as much room to allow for extra charge.

I use a Dillon 650 now with the powder check on it.   Periodically spot check my powder in the case.    Every thing runs fine and never have a problem.   Only had it happen once, once was enough.

I won't talk about the day a 10mm round found it its way into my .45 acp ammo bag and how the round fed perfectly in my Sig P220 and it fired fine too.   Didn't eject since the cartridge looked like lace after it had expanded to fit the chamber.  It didn't just crack, the entire case exploded to the circumference of the chamber.   I now use separate ammo bags for my 9mm, 10mm and .45 acp.   So there is less danger of cross contamination.

My single biggest fear is a 300 blackout round (or loaded mag) making it into a 5.56 AR.  They can and do chamber, but the results of firing are bad, bad bad.

I have a Dillon 650 set up with the automatic case feeder and a Mr. Bullet bullet feeder.  I also have a powder check alarm at my 3rd station, but I'm going to move it off, and replace the Dillon Crimp+Seater die and the open station with a Redding Micrometer adjusted Seater and a Redding Micrometer adjusted taper crimp die.  It takes entirely too long to get the seating depth adjusted precisely with the gross movement the die threads provide.  I have a strong light on the station right after the powder dump, and I'll be able to see a double charge - it'll be running over the sides of the brass.  To answer the general question, I've been fortunate, no double charges or destroyed guns.
