Hornady Lock-N-Load powder measure rotor assembly just fall out unexpectedly

Any of you had the Hornady Lock-N-Load powder measure rotor assembly just fall out unexpectedly when dispensing a charge?. The issue is when the rotor falls out powder goes everywhere. This has only happened twice in three years.
This unit works well, as I verify each charge on the GemPro 250, as long as it doesn't just come out that is

Is the o ring intact?  The handle keeps it from falling out of the other side
Actually the meter Assy keeps the rotor from falling out
